- Complete a Registration Form which may be
obtained in the following ways:
- Forms are located in the church, on
either side of the front entry doors
- Obtain one from the parish or
school office

Print out PDF
- Once you have completed the form, you may
give it to an usher at Mass or turn it into the
parish or school office.
Please note that if you are already a registered
parish-ioner within the Diocese of St. Petersburg,
you are only allowed to be registered at one parish.
